At a Special Term of the
County Court held in and for
the County of Monroe at the
Court House thereof in the
City of Rochester, State of
New York, on the 17th day
of April, 1939.
Present: Honorablo H. Douglasa
Van Duser, County Judge.
In the Matter of the Application of
Frank L. Stevensky for leave to
assume the name of Frank B.
Stevens.
The petition of Frank L. Sterensky
duly verified the 15th day of April,
and the consent of Nora M. Stevensky,
his wife, and entitled as above,
having been duly presented to the
County Court of Monroe County, the
said petition praying for leave of
the petitioner to assume the name
of Frank B. Stevens in place and
stead of his present name, and the
Court being duly satisfied that the
averments contained in such petition
are in all respects true and that
there is no reasonable objection to
the change of name proposed.
Now, on motion of Albert E.
Avery, attorney for the petitioner,
it is
Ordered, that Frank L. Stevensky,
of 75 Lapham Street, Rochester, New
York, be and is hereby authorized
to assume the name of Frank L.
Stevens on May 22, 1939, upon condition,
however, that he shall comply
with the further provisions of this
order, and it is further
Ordered that this order and the
uforementloned petition be filed within
10 days from the date hereof in
the offlee of the Clerk of the County
of Monroe; and that a copy of thia
order shall within ten days from the
date hereof be publiahed once In t he
Daily Record, a newspaper published
in Rochester, New York, and that
proof of publicatlon thereof be filed
with the County Clerk of Monroe
County, and it is further
Ordered that following the flling
of this petition and t h e order herein
and the publication of .such order
and the filing of proof of publication
thereof a.s herelnbefore direeted, and
on and after May 22, 1939, the petitioner
shall be known by the name
of Frank L. Stevens and by no other
name.
